Fork, 26-5-1-1-1-responding. 265 Colorado Avenue. The 65-year-old female pain in her left shoulder and chest pain was in the hospital last night for the same thing. She does have muscular and nerve pain as well. Started Wednesday and been getting lost ever since. Had a CT done and was diagnosed with frozen shoulder. No history of cardiac issues still obtaining.
