Chief complaint is going to be stroke alert.

Patient's sugar was 136.

Patient last had a stroke approximately September 4th.

About a month and a half ago, patient has is unable to track with his eyes and has speech changes.

Patient does have left-sided deficits from the previous stroke as well.

Last known normal was 12:00 p.m.

confirmed yesterday and likely passed that with the staff.

Oncoming crew noticed at about 08:00 a.m.

this morning that he was not acting normal and not able to speak to them.

So we'll be transporting emergency van.

Yeah, last pressure was 105 over 70 , heart rate 60 , SpO two is 100 on room air.
