We're about sent out from your facility and route with a (number withheld) female who's coming from home.

Chief complaint today is going to be right lower leg pain.

Patient had a surgery with an external fixator put in on her lower leg on Sunday.

She was discharged from your facility today and sent home.

However, she does not have any pain control out of it.

And getting home, she experienced another fall with it and she doesn't feel like she's able to perform any activities of daily living safely at home.

After talking with the home health nurses, they also advised that they called and tried to return to the hospital until she can safely do that.

Her vital signs are sinus on the monitor at 80.

She's 95% on room air.

Blood pressure is 157 over 92.

And we got a GCS of 15.

She's got no other real trauma or novel injuries.
