Patient's chief complaint today is going to be a fall.

Patient experienced a fall about 07:00 this morning, rolled off the bed.

We are coming from a nursing home.

Staff states they are concerned because the patient became altered.

Right, prior to calling us, probably about 15 min .

But since being with us, he is a GCS of 15 , answering everything appropriately.

He is on a blood thinner, he takes Eliquis.

Not complaining of any pain.

Current set of vitals, I got a pulse rate of seventy-two.
