We're non-emergent with a sixty-eight, sixty-eight female coming from a skilled nursing facility.

Chief complaint is going to be altered level of consciousness for the facility staff.

They were unable to tell me what her last known normal was or even give me an approximate time.

She's normally a GCS of 15 .

She's currently a GCS of 14 .

She is supposed to wear a baseline oxygen at three liters through a nasal cannula.

It was not on her upon our arrival, and she was at 70 % room air.

Back on her baseline oxygen, now she's at ninety-eight.

Got an end tidal of 50 .

Respiratory rate of 18 .

Lungs are clear and equal bilaterally.
