Chief complaint of shortness of breath has been ongoing for about one to two weeks.

Currently alert and oriented times four.

GCS of 15. Vital signs as follows: heart rate of 66, blood pressure of 215 over 81, breathing about 18 times a minute with a no two sat of 99% on bleeders by nasal cannula.

Sinus rhythm on the monitor.

Does have a history of CHF and does have some bilateral crackles on her lung sounds.
