This is Tower. My minute fifty-six.
00:01
I see a down report.
00:02
Seventy-one year old male with a fall.
00:05
Go ahead.
00:08
Good morning, Palomar 756.
00:11
We have a 71-year-old male status patient.
00:15
We're going to be going to your destination for a special resource due to age and then fall with no defensive measures at the time of fall.
00:25
It's going to be PCR number (number withheld).
00:30
(number withheld).
00:34
We were called out to the Spencer Elementary School up here in the Julian area when my patient was walking and around with some kids.
00:43
One of the kids fell down and my patient, the one-year-old male, tripped over the child that they were running around with, and he was not able to go and put his hands out.
00:54
Fell straight onto the left side of his body, including the left side of his head.
01:00
The patient denied any dizziness prior to the fall.
01:03
He states that he just could not stop in time and tripped over that child from a standing position.
01:08
He was not an elevation in any, in any spot higher than 6 ft .
01:14
He was at ten, and he's about five nine.
01:19
There was no loss of consciousness, but there was some dizziness and a little bit of haze after the injury.
01:24
But never full
01:26
loss of consciousness.
01:27
Upon our arrival, patient was speaking full sentences.
01:30
GCS of 15 .
01:31
Recalled the whole event and with a detailed history of everything that happened up until that point.
01:39
We went ahead and placed this patient in modified spine on scene, placed him on our gurney, and loaded him up to our gurney.
01:46
Once we placed him in the ambulance, started 20 gauge
01:50
IV in the left AC.
01:52
Went ahead and started capnography to monitor due to the head injury, and then also gave him 1 g of Tylenol for the pain on his head, which was a three out of ten, then shoulder as a nine out of ten at this time.
02:05
So further on down, we went ahead and gave 15 .
02:07
Micrograms of fentanyl to go ahead and assist with that pain.
02:12
At this time, the patient didn't lose any consciousness.
02:15
He's complaining of any other dizziness, chest pain, nausea, vomiting, or anything like that.
02:22
He's able to go ahead and still tell us recall while they are.
02:25
His pupils were normal.
02:26
Everything is looking great on this patient.
02:28
We are about a 15 min ETA to your destination.
02:32
Do you need anything else?
02:35
Just to clarify, nine out of ten shoulder pain.
02:39
I'm assuming it's on that left side that fell on.
02:42
Any outward signs of trauma to the head?
02:46
That's affirmative.
02:48
It's going to be about a three-inch laceration to the left side of his forehead, right above the left eyebrow.
02:55
Bleeding is controlled and shut at this time.
02:59
We arrived and bleeding was controlled once we arrived on scene.
03:04
Do you know if this patient takes any blood thinners?
03:08
My apologies, negative.
03:10
No blood thinners at this time for this patient.
03:13
And lastly, any deformities to that shoulder?
03:18
Negative. Just pain on palpation and movement.
03:23
Copy that, 5640.
03:24
With your special consideration upon arrival, you're clear with Palomar 1125.
