Heard on:
San Diego CA BLS SLJ (Hospital)

00:00
This is Air Care BLS Unit One Hundred and Ninety.
00:08
Copy for a medical direction from El Cajon Medical Control.
00:14
Copy that. Air Care, which hospital are you guys looking to go to?
00:21
We have an ETA of 12 minutes to Tri City Hospital, but we also have an ETA of 17 minutes to Palomar.
00:27
Family is against going to Tri City, but we wanted to get medical directions just to see if we can in fact transport to Palomar instead for the longer ETA.
00:42
I copy. You're a little broken at the end.
00:44
Go ahead.
00:46
Thank you. Our run number is 230114 .
00:52
We have an eighty-four year old female patient with a chief complaint of deviation from baseline.
00:59
Patient's A and O baseline is A and O times three.
01:03
Patient's A and O baseline on scene is A and O one.
01:08
Vitals are stable and within normal limits.
01:13
Blood pressure is elevated at 50 over 80 , which per the med tech is a bit high for her.
01:20
Patient also does have a blood sugar of 200 and sixty-nine.
01:24
Break.
01:25
So basically, she was last seen normal around 10:00 a.m.
01:30
this morning.
01:32
Around 11:00 , after lunch, she didn't really eat too much, wasn't really too hungry.
01:38
She began to become lethargic, and her status began to deteriorate.
01:43
Her A and O one from there.
01:45
Patient is normally ambulatory with a walker, but now is unable to bear weight on her own and now is in a wheelchair.
01:53
At one point, staff did report that she became lethargic and very, very sleepy, and she was able to respond with very light physical stimuli, and would only open her eyes to that.
02:07
Patient is a type two diabetic and also has a history of chronic UTIs as well.
02:13
One of the signs she's been showing is moving her jaw side to side, which per the family is a sign that she has a UTI.
02:22
The rest of the vitals is just the temperature of ninety-eight .6 , heart rate of 80 , satting at ninety-six on room.
02:30
And then again, we have an ETA of about 12 min to Tri City and an ETA of about 17 to Palomar.
02:37
The family does not want us to go to Tri City, but.
02:41
We need to go to the disorder with the closest ETA.
02:44
We will, we just want a medical direction in regards to that.
02:48
Are you guys still on scene?
02:56
Correct, we are still on scene and also our patient is negative on PFAST.
03:04
I know the BFAST is negative, and she has a history of UTIs and could possibly see what's going on.
03:12
But the fact that her baseline has dropped
03:15
A and O times three to one, and she's now unable to walk when she normally can,
03:21
I would almost upgrade this to an ALS call.
03:26
Copy that. We will go ahead and upgrade.
03:29
Thank you.
