Yesterday patient fell approximately 1500 hours in his room.

He fell backwards from a standing position, striking the back of his head, and his upper back were patched in.

Patient was complaining to the nursing staff that it's hard for him to hear.

He's having some hearing loss problems after his fall.

Patient is alert and oriented.

Times four has a GCS of 15 .
