She's complaining of an altered mental status.

We're looking at a GCS of like 13 to 14 roughly.

She is tracking her environment to verbal response.

And able to give me like a thumbs up here and there.

Patient has a history of anxiety.

Vitals are as followed: sixty-five sinus on the monitor.

Main issue is that mom states that she fell, hit her head approximately an hour ago, with a momentary loss of consciousness.

States about a few seconds.

Patient was normal afterwards and then went unresponsive for her.

We have her in a C collar at this time.

No signs of trauma.

Noted blood sugar is 103 .
