Chief complaint that there's going to be altered mental status.

Patient is currently having a UTI and is being treated by antibiotics.

Nurse on the scene stated he is not his normal self.

He has not been his normal self since he's been at the nursing facility.

His GCS is about 14.

He is kind of out of it.

He will respond to me if I talk to him, but he's not like spontaneously like alert.

Patient does have a defibrillator.

Got a set of vitals at this time.

Heart rate's going to be 87, SpO2 is 98% on room air.

Respirations are about 29, labor.
