Yeah, resident medic 615 Carole.
00:01
I am inbound to your facility with a seventy-two year old female.
00:05
She's complaining of chest pressure.
00:07
Said it began this morning when she woke up, but also advised that she's been experiencing lethargy and just increased weakness over the last week.
00:15
She said she did have a heart attack back in 2015 with consequential heart failure.
00:20
She says that the chest discomfort though had begun when she woke up at 06:00 a.m.
00:27
She does say that it is positional; she feels better when she's sitting upright.
00:31
But the pressure, she says, started at nine out of ten and while sitting upright is now at a seven out of ten.
00:38
She's alert, oriented, GCS of 15 , twelve.
00:42
Twelve lead was unremarkable and no signs of ST elevation.
00:45
The sinus tach at a rate of 108 .
00:48
Her oxygen saturation began at 94 % on room air, but did begin to decrease to about 90 % .
00:54
So I do have her on two liters of oxygen via nasal cannula now.
00:58
Last blood pressure was 170 over 151 .
01:02
I did get a sugar of 04:11 .
01:05
So I do have a 20 gauge IV established in the left AC that I am giving her some normal saline to try to help with the sugar.
01:11
She has 324 of aspirin and she is dissolving one nitroglycerin as we speak.
01:17
I don't know if you have anything further.
01:19
We're going to be there in about seven more minutes.
