We're going from Twin Oaks assisted living of an eighty-seven year old female.

They called out of concern of an altered mental status.

They reported ground level fall of some sort at approximately 02:00 p.m.

earlier today.

They report they came and found her sitting on the floor.

Best we can gather, they found no injuries, no issues.

From that point, this evening, the patient was noted to have an increased altered mental status.

They report sometimes she is altered in the evening, but tonight was worse than normal.

She is alert to self in the current surroundings.

Last set of vitals: pressure 100 and thirty-four over seventy-three, afib.

History: atrial fibrillation, irregular rate in the eighties at ninety-five percent on room air.

She's in mild speed.

We have no injuries, no complaints.

Normal temp, normal blood sugar.
